Important Information Regarding COVID-19

Moments Hospice remains committed to our mission of Changing the Hospice Experience One Moments at a Time. We also remain committed to taking care of our staff, patients, families and referral partners. We wanted to provide an update on what we’re doing to help mitigate the spread of the COVID-19 Virus.

Education

All Moments Hospice employees have received education from the CDC, WHO, and CMS on preventing the spread of COVID-19 and are screened prior to patient visits according to prescribed CDC/WHO guidelines. All TEAM members will also be required to take a series of online educations regarding COVID-19 and the CDC, WHO and CMS direction on how to protect our staff, patients, families and referral partners.

Secondary Screenings

All Moments Hospice employees willingly submit to secondary screening protocols per partner facility guidelines. And we will follow any and all Skilled Nursing, Hospital, and Assisted Living guidelines.

Updates and Training

All Moments Hospice employees are updated daily regarding CDC/WHO/CMS recommendations along with specific training through the National Hospice and Palliative Care Organization (NHPCO).

Our Compliance/QA officer currently attends 5 separate emergency coalition meetings which represent all of our geographic service areas in MN and WI. As well as attends the weekly Minnesota Department of Health (MDH) updates regarding the COVID-19 Virus.

Our Commitment

Moments Hospice is committed to consistent staffing for maximum safety, comfort, and overall patient experience. We will ensure that our staff has the hours needed to maintain their jobs and provide for their families. We are also focused no keeping our TEAM members, Families, Patients and Referral Partners Safe.

Patient Care

Moments Hospice remains committed to a timely and seamless transition for new referrals as well as confident in admitting and caring for those specifically diagnosed with COVID-19.

Virtual Care

Moments Hospice offers multiple alternatives for ongoing support: virtual care visits by MSW/spiritual care/Bereavement support, educational support across technology platforms.

Volunteer Visits

Out of an abundance of caution, Moments Hospice is limiting volunteer visits for the next two weeks to mitigate the possibility of community exposure of COVID-19.

DME Vendors

Moments Hospice contracted DME vendors operate under strict protocols established by CDC/CMS and will coordinate according to facility ,or family instructions.
