About Moments Palliative Care

We recognize the unique stress and hardship advanced disease cause patients. We strive to extend the highest level of courtesy and service to patients and families/caregivers, community partners, and each other.

What is Palliative Care?

According to the Center to Advance Palliative Care (CAPC): “Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve quality of life for both the patient and the family.”

Initial Moments

At Moments, we schedule visit at home, assisted living, skilled nursing, or other residence with a Nurse Practitioner. During that visit the provider will assess the need for therapy services and/or visits from the chaplain to assist with overall wellness of patient and family. Our visits typically last more than 45 minutes to discuss medical history, any unmanaged illness or symptoms, disease trajectory, and advanced care planning. With an understanding of each patient’s needs and goals, we help develop an individualized plan of care.

Supportive Care

Navigating the healthcare system can be difficult. Moments is pleased to offer the continuum of care to our patients. Our Palliative Care team is able to help discuss healthcare options, request therapies, and order tests or diagnostic studies while the patient stays in the comfort of their own home.

Meaningful Care

Listening to patients and families about what is most important helps shape the plan of care. Each goal is selected to make a meaningful impact on patient’s quality of life. As new information or preferences are discussed, the goals may change but always respectful of patient’s wishes.

Coordination of Care

Palliative Care works with all other providers in a patient’s care team, including:

  • Home Health
  • Specialists such as Oncology and Cardiology
  • Pain management centers
  • Assisted Living and Skilled Nursing staff
  • Primary Care providers
